In order to participate in Wilson Athletics, a student must complete the online clearance process.

Directions for the Online Athletic Clearance Process


Summer Sports Physical Waiver

If you are participating in a summer camp, you must complete the summer sports physical waiver and submit to the coach prior to participating.

Physical Exam

Students must have a physical examination by a medical doctor.


The completed physical examination form must be uploaded when completing the online clearance process and submitted to the Athletic Secretary, Colleen Walker in the main office.

Suggested Locations for Sports Physical


A physical is valid for one year from the date of the exam.

Medical Insurance Required

In order to participate in athletics, all student-athletes must have valid medical insurance coverage. If you do not have current medical insurance, you may purchase a plan through Myers-Steven Insurance. Please see the Athletic Secretary in the main office to pick up a form.

Summer Athletics Physical Waiver

In order to participate in a summer camp, students and parents must complete the summer sports waiver packet. This summer waiver packet must be turned in to the head coach prior to participation.
